In-House Hospice has been providing high quality hospice care for over 12 years. With 5 offices in Michigan and one in Ohio we provide an interdisciplinary approach to care wherever the patient resides. Each patient receives individualized care that includes, Medical Director, Registered Nurse Case Manager, Master Level Social Work, Chaplains, Certified Nurse Assistants, and some patients also receive Massage Therapy and Music Therapy. Our service levels are higher than other hospices and we provide full time After Hours staff because we realize that end of life needs don't occur Monday through Friday during the day. Most patients receive between 5 and 7 visits per week of service.

IN-HOUSE HOSPICE JOB DESCRIPTION
HOSPICE AIDE
POSITION SUMMARY
Under the direction of an RN, provides personal care services within the guidelines of the IDG Plan of Care to patients enrolled in the IN-HOUSE Hospice program.
MINIMUM QUALIFICATIONS
Current State Nursing Assistant certification
Previous hospice experience preferred
High school diploma or equivalent preferred
PHYSICAL REQUIREMENTS
Able to read 12 point or larger type
Able to hear and speak in a manner understood by most people
Communicate effectively in English both orally and in writing sufficient to prepare and send reports, and to interact with clients, staff and peers (proficiency in a second language which corresponds to the language of a significant portion of the patient population is helpful)
Able to stoop and bend
Able to lift at least 25 pounds and bear the weight of an average adult effectively so as to be able to perform the above listed functions
Able to travel to nursing facilities where patients reside
Reliable transportation with required insurance coverage
RESPONSIBILITIES AND FUNCTIONS
Under the direction and ongoing supervision of an RN, the aide will provide personal care assistance/ADL’s, companionship and emotional support within the guidelines of the IDG care plan to patients and families/caregivers enrolled in the In-House Hospice program.
The Hospice Aide:
does not administer medications of any kind
will maintain required competency and skills
will maintain current licensure as required by the State
maintain current knowledge and practice of infectious disease protocols
may be required to work an on-call weekend rotation to meet the needs of patients 24/7
